Jason Crouch, D.O. is board-certified, specializing in Family Medicine. He was born and raised in Chandler. He is a proud graduate of C.H.S and a Valedictorian of his class. He received his medical degree from OSU College of Osteopathic Medicine after earning his B.S. degree in Cell and Molecular Biology at Oklahoma State University. Following medical school, Dr. Crouch completed a 3 year internship and residency in Family Medicine at OSU Medical Center in Tulsa. Dr. Crouch chose the specialty of Family Medicine due to the variety of patients and conditions he could treat. Throughout the three-year residency, Dr. Crouch rotated through several medical disciplines including: Pediatrics, Adult Medicine, Obstetrics, Gynecology, Orthopedics/Sports Medicine, General Surgery, Rheumatology, Radiology, Emergency Medicine, Internal Medicine, and in-patient hospital and outpatient clinical care. Upon completion of his residency, Dr. Crouch was proud to be named Outstanding Resident of the Year in Family Medicine.

Dr. Crouch practices the full scope of family medicine including Adult Medicine, Pediatrics, Women’s Health, Sports Medicine, and minor emergency care. He is a member of American Osteopathic Association (AOA), the Oklahoma Osteopathic Association (OOA), and the American College of Osteopathic Family Physicians (ACOFP). He is Board Certified by the American Osteopathic Board of Family Physicians (AOBFP).

Dr. Crouch chose to return to his hometown to open his private practice due to the positive, lasting impression the people of the community have had on his life.

When not in the office, he enjoys spending time with his spouse, Robin, and daughter, Tatum.
