Thank you for choosing Dr. Crouch for your health care needs. During your first visit, you will meet our staff, complete a few brief forms and, of course, meet Dr. Crouch. As a family practice, we will try to solve your current medical problem and detect or prevent other health problems. We hope to make the first visit not just an opportunity to deal with any medical concerns you may have but also a time to get acquainted with you and speak to you regarding preventive medicine specific to your needs.

Please bring all of your current mediciations or a list of all current medications to your first appointment.

Please also arrive 10-15 minutes early to allow time before your appointment for completion of our new patient paperwork.
