Crouch Family Medicine is the private, solo practice of Jason Crouch, D.O. It is privately owned and operated by Dr. Crouch. The practice opened in Chandler, Oklahoma on July 30, 2007. Unlike many medical practices in the U.S., Crouch Family Medicine is not owned or managed by any hospital or hospital group. This gives Dr. Crouch the freedom to refer you the specialist of your choice. Dr. Crouch answers to himself and his patients.  He is the ultimate decision maker for his practice. There are many good hospitals in the area and Dr. Crouch is happy to refer you to the hospital and specialist of your choice for your medical need.

The clinic is a non-emergency facility offering lab services, Digital X-Ray, EKG, and spirometry.
